We are actively recruiting for a hospital in northwestern Alaska that is looking for an Activity Director. The hospital strives to provide its colleagues with a positively charged work environment, treating them with respect and appreciation.

The Chronic Disease Case Manager will promote health care delivery across the continuum that will enhance the client’s quality of life, decrease fragmentation of care, while efficiently utilizing resources and containing cost. They will provide resource support to the Case Management Team. The Chronic Disease Case Manager will use a process that includes screening and case finding, comprehensive multidimensional assessment, care/discharge planning, connection with available resources, implementation of the plan, and ongoing monitoring and re-assessment. The plan sets goals and time frames for achieving goals that are appropriate to the individual and his/her family and are agreed upon by the family and the treatment team. This position reports directly to the Outpatient Services Program Manager.

Develop programming, write care plans, do assessments, recruit volunteers, 1:1 individual as well as group programs.

Computer- software system is Point Click Care.

Shift or schedule requirements.
8:30-5 pm. Evenings and weekends as needed.

Floating to other units/departments.

Bed size/number of rooms for this unit/dept.

Dress Code: Business casual

Tell us about the unit/department specific training and orientation program.
Will go through Maniilaq and LTC orientation. Will share previous programming information as to what has worked and what hasn’t.

Additional Comments:
Programming will be different than many lower 48 facilities. Elders are unique and programming challenges exist due to the unique location of the facility (above the Arctic Circle). Rewarding professionally in taking care of these individuals who are the last of their kind growing off of the land.

Requirement Note

At least 2 years of experience in activities. Certification preferred. COTA acceptable. Able to work with other cultures and Elders
